Running a team in Charlotte, Raleigh, or Wilmington also means running a small back office. Someone has to cut checks, answer benefits questions, and keep personnel files straight. The state has 1.1 million small businesses and 1.8 million people on their payrolls. No wonder the search for a North Carolina PEO is so common.

3 facts shape that search. The state income tax keeps falling: it is a flat 3.99% in 2026, and it’s already set to drop to 3.49% in 2027. Workers’ comp applies from 3 employees, with corporate officers counted. Health benefits are expensive for a small group. On the plus side, the state licenses PEOs, which gives you a quick first check.

PEO North Carolina: Why Small Businesses Use One

Payroll here is lighter than in many states. No city or county taxes income, and the state rate is a flat 3.99% for 2026. The catch is that the rate won’t stay the same. S.B. 257, signed on July 7, 2026, lowers it to 3.49% for 2027 through 2029, then to 3.24% and later 2.99%. Every step brings new withholding tables from the North Carolina Department of Revenue (NCDOR). The annual NC-3 reconciliation with W-2s stays in place. A North Carolina payroll service earns its fee by keeping withholding current through each scheduled change.

INSIGHT: What is changing in North Carolina payroll tax?

The state rate falls on a fixed schedule. Withholding has to be updated at the start of 2027 and again in later years.

  • 2026: flat 3.99%, down from 4.25% in 2025.
  • 2027 to 2029: 3.49% under S.B. 257.
  • From 2033: 2.99%, after a step to 3.24% in 2030.
  • Local income tax: none.

Employers have separate duties. Employers must pay unemployment insurance (UI) tax on the first $34,200 of each employee’s 2026 wages, and only the employer pays it. New employers start at 1%, and the Division of Employment Security (DES) assigns a rate after that. Wages are reported to DES every quarter. New hires must be reported within a 20-day window. State withholding deposits and federal FICA and FUTA follow their own dates. Providers of payroll services in North Carolina run that calendar for you, and so does a PEO.

2 things make owners look past payroll. The first is workers’ comp. It is required once you have 3 employees, and corporate officers count, so that a small corporation can cross the line quickly. No state fund exists. Coverage comes from a private carrier, self-insurance, or the assigned risk plan, and a PEO can supply it through its own program. The second is benefits. Pooled health, dental, vision, and a 401(k) are hard to match on their own. Health coverage is optional below 50 full-time-equivalent employees, but it is the first thing a job candidate raises. Those 2 needs explain the appeal of North Carolina PEO services.

INSIGHT: Which insurance and benefits does a North Carolina employer have to provide?

Workers’ comp is mandatory for employers with 3 or more employees, including officers. Nothing forces a small firm to offer health or retirement plans, though recruiting suffers without them, and pooled PEO plans narrow that gap.

  • Workers’ comp: 3 or more employees, counting corporate officers.
  • Radiation work: coverage from 1 employee.
  • Federal ACA duty to offer coverage: from 50 full-time-equivalent employees.
  • Not required by the state: disability insurance, paid family leave, a retirement plan.

Which model fits you? A PEO becomes a co-employer and combines payroll, benefits, workers’ comp, and HR, yet you still manage your people. With a payroll service, the job ends at paychecks and tax returns. A third model, HR outsourcing with no co-employment, falls in the middle. My article on what a PEO is explains the trade-offs. Anyone selling PEO services in North Carolina needs a license from the Department of Insurance (NCDOI). The department publishes its list of active licensed PEOs, and the IRS posts its own list of Certified PEOs (CPEOs). Check both before you sign.

I reviewed 15 options for local employers. Some are PEOs based here, some are national PEOs licensed in the state, and a few are brokers or advisers that help you compare. Each entry says plainly which kind it is. If you would rather pick a North Carolina PEO provider from a national field, see my guide to the best PEO companies.

Top 15 PEO Companies in North Carolina

Before choosing a North Carolina PEO company, settle 3 points. Is the goal a full bundle of payroll, benefits, and workers’ comp, plain payroll with tax filing, or HR advice with no co-employment? How many people do you employ, given the 3-employee workers’ comp threshold and the ACA mark at 50? Finally, does an in-state office outweigh the bigger benefits menu of a national PEO?

I judged the PEO companies in North Carolina on a short list of criteria. An active NCDOI license came first. Next were accurate withholding through the scheduled rate cuts and on-time DES and new-hire filings. I also looked at the workers’ comp model, health and 401(k) plans, and HR and handbook support. CPEO status or accreditation where claimed and clear pricing finished the list. The A-to-Z order is deliberate.

All 15 providers appear in A-to-Z order. The sequence says nothing about quality, preference, or ranking.

How Much PEO and Payroll Services Cost in North Carolina

PEOs usually bill a flat amount per employee per month or a percentage of total payroll. Only Justworks puts PEO rates on its site: PEO Basic at $79 and PEO Plus at $124, per employee per month. Payroll-only plans cost much less. Among payroll services, North Carolina owners can price online; Justworks charges $8 per employee plus a $50 monthly base. CoAd starts at $55 a month plus $5 per employee. Neither plan includes benefits or workers’ comp.

The fee for a PEO in North Carolina normally covers payroll and tax filings, benefits administration, workers’ comp administration, and HR support. You pay separately for health and workers’ comp premiums, the 401(k) plan, and onboarding. Statutory costs are yours either way. You owe UI tax on the first $34,200 of wages, at 1% for new employers in 2026, plus federal FICA and FUTA. Compare the total yearly cost, not the headline fee.

INSIGHT: What should North Carolina payroll services handle for you?

Payroll here is simple on paper. The risk lies in rate changes and recurring filings, so check how a provider tracks each.

  • Withholding at the flat 3.99% rate, with 3.49% scheduled for 2027.
  • Employer UI tax, capped at $34,200 of wages per person.
  • Quarterly wage reports to DES.
  • New-hire reports no later than day 20.

Plenty of Carolina companies have a salesperson or a second office in Florida. Florida has no state income tax, though unemployment and workers’ comp follow different rules. My guide to PEO companies in Florida covers the providers licensed there.

Remote hires in other states can change the math more sharply. California brings state disability insurance, daily overtime, and paid sick leave. If your team reaches the West Coast, my overview of PEO companies in California is the next read.

FAQs on PEO and Payroll Services in North Carolina

What does a PEO do for a small business in North Carolina?

A PEO takes on part of the employer role through a co-employment contract. In practice, it runs payroll and files state and federal employment taxes for your staff. It opens the door to group health and retirement plans and provides workers’ compensation coverage under its program. It also handles routine HR paperwork, from onboarding forms to handbook updates. You continue to decide who is hired, what they do, and who leaves. The service agreement sets out the split of responsibilities in writing.

Do PEOs need a license in North Carolina?

Yes. The Department of Insurance licenses these firms under Article 89A of Chapter 58 of the General Statutes. The department offers a full license, a group license for companies under common control, and a limited de minimis registration for certain out-of-state firms. The department publishes a list of active licensees, so you can look up a provider before you sign. Check the exact legal entity named in your contract. IRS certification is a separate federal status and does not replace the state license.

When does a business in North Carolina need workers’ compensation?

Generally, once it regularly employs 3 or more people. Corporate executive officers count toward that number, although they may exclude themselves from the policy. Certain work, such as jobs involving radiation, requires coverage from the first employee. The state has no insurance fund. Coverage comes from a private insurer or approved self-insurance, and employers who cannot find a carrier use the assigned risk plan. A co-employment provider’s program lets many small firms get covered without their own policy.

What is the state income tax rate in North Carolina for 2026?

The individual income tax is a flat 3.99% for 2026, down from 4.25% in 2025. A law signed in July 2026 lowers the rate to 3.49% for 2027 through 2029. It then drops to 3.24% in 2030 and to 2.99% in 2033. No city or county levies an income tax, so employers withhold only state and federal tax. Each rate change requires new withholding tables, and payroll systems should be updated before the first paycheck of the new year.

How much does a PEO cost in North Carolina?

Pricing takes 1 of 2 forms: a monthly fee for every employee or a percentage of total payroll. Most providers quote individually based on headcount, industry, and the benefits you choose, though a few publish per-employee rates. Beyond the service fee, expect charges for health premiums, workers’ compensation premiums, and retirement plan costs. Request an itemized proposal from each provider. Then compare your total cost over a full year, because the advertised fee is only part of it.

What is the minimum wage in North Carolina?

The state minimum wage is $7.25 an hour, the same as the federal rate. Employers may pay tipped employees a cash wage of $2.13 an hour, provided tips bring them to at least $7.25. If tips fall short in any week, the employer makes up the difference. Overtime is due at 1.5 times the regular rate after 40 hours in a workweek. The state has no paid sick leave requirement, so each employer’s handbook and practices determine time-off policies.
