Whether you run a lab in Cambridge, a contractor in Worcester, or a restaurant group in Boston, the back office keeps growing. The state has 756,096 small businesses employing 1.5 million people, and most face the same squeeze. A Massachusetts PEO is one way out, and the state makes vetting easier: PEOs must register here, and the list is public.

3 things weigh on a small employer. Payroll carries Paid Family and Medical Leave (PFML) contributions, set at 0.88% of eligible wages for larger employers in 2026, plus several unemployment assessments. Paid sick time and workers’ comp rules reach even the smallest teams. And health benefits cost a small group far more per head than a large one.

PEO for Massachusetts Small Businesses: Why Owners Use One

A pay run here has many lines. Income tax is a flat 5.0%, with a 4% surtax on income above $1,107,750, and no city adds its own. PFML contributions come out of wages. Employers also owe contributions and assessments to the Department of Unemployment Assistance (DUA). Add earned sick time accrual and a $15.00 minimum wage. None of it is hard alone. Together, each program has its own rate and rules on every pay run. That is why the payroll services Massachusetts employers buy tend to earn their fee.

INSIGHT: What does a Massachusetts payroll include in 2026?

Beyond income tax, payroll here carries PFML and several employer contributions. It has more lines than most states.

  • Income tax: flat 5.0%.
  • PFML: 0.88% of eligible wages for employers with 25 or more covered individuals.
  • Unemployment insurance: first $15,000 of wages, 2.42% for new employers.
  • New construction employers: 6.08% on the same wage base.

as of October 2026

The employer side is where the detail sits. The state’s unemployment insurance (UI) charge falls on the first $15,000 of each employee’s wages. The same wages carry a Workforce Training Fund contribution, a COVID-19 Recovery Assessment, and, for many firms, the Employer Medical Assistance Contribution (EMAC). DUA wants a report every quarter. PFML has its own filing, and new hires go to the Department of Revenue (DOR) within 14 days. Uncle Sam’s FICA and FUTA are added to that. Buyers of Massachusetts PEO services, payroll included, hand this whole calendar to the provider.

Insurance and leave rules explain the step up from payroll to a PEO. Every employer must carry workers’ compensation for its employees, and no state fund exists. A PEO can provide coverage through its own program, and state law lets the agreement specify who provides it. Earned sick time applies to every employer and must be paid once you have 11 employees. In a labor market this competitive, pooled medical, dental, and vision plans, plus a 401(k), carry real weight. Those 3 pressures drive most demand for Massachusetts PEO services.

INSIGHT: What does a Massachusetts employer have to provide?

Workers’ comp, earned sick time, and PFML apply to small employers too. A PEO can take on the administration.

  • Earned sick time: 1 hour per 30 worked, up to 40 hours a year, paid at 11 or more employees.
  • PFML: 0.46% for employers with fewer than 25 covered individuals, with no employer share.
  • Minimum wage: $15.00 an hour; tipped service rate $6.75.
  • Federal ACA: coverage must be offered once you reach 50 full-time-equivalent employees.

Your real choice is among 3 services. A PEO co-employs your staff and combines payroll, benefits, workers’ comp, and HR, yet you keep running the business day to day. A payroll service issues paychecks and submits the filings. The in-between option is HR outsourcing, with no shared employer status. See my guide to what a PEO is for the contract terms. Regulation is strict. A PEO in Massachusetts must register with the Department of Labor Standards (DLS) every year and post a $250,000 bond. Its clients must notify employees about co-employment. DLS publishes its list of registered PEOs, and the IRS publishes its Certified PEO (CPEO) list.

I reviewed 15 options for employers in the state: providers with a local address and national PEOs that serve it. For every name, you will see what it does, how it charges, and whom it suits. If you’re researching PEO Massachusetts options alongside a nationwide search, you can also read my guide to the best PEO companies.

Top 15 PEO Companies in Massachusetts

The PEO companies in Massachusetts on this list are not interchangeable, so ask 3 questions first. Is a full package the goal, with benefits and workers’ comp tied to payroll? Would payroll and filings be enough? Or is HR advice all you lack? How many people do you employ, given the paid sick time line at 11, the PFML line at 25, and the ACA mark at 50?

Then decide between a firm based here and a national provider with a larger benefits program. There is no single best PEO company in Massachusetts, only the one that fits those answers. Size, industry, and your tolerance for paperwork all change the result.

I judged all PEO service providers in Massachusetts by the same criteria. I ranked current DLS registration first, followed by accurate PFML and DUA handling and earned sick time tracking. Next come workers’ comp arrangements, medical and 401(k) plans, and handbook and HR support. Any CPEO or accreditation claim and open pricing finish the list. The order is alphabetical on purpose.

All 15 providers appear in A-to-Z order. The sequence says nothing about quality, preference, or ranking.

How Much PEO and Payroll Services Cost in Massachusetts

PEOs bill in 2 ways: a flat fee per employee per month, or a percentage of total payroll. Most quote individually. Justworks is the exception, at $79 per employee per month for PEO Basic and $124 for PEO Plus. Stand-alone payroll services in Massachusetts cost much less, priced per run or per employee, but they include neither benefits nor workers’ comp. That PEO fee generally covers payroll and filings, benefits and workers’ comp administration, and HR support.

*As pricing is subject to change, we are listing prices as they stand in October 2026

We charge health premiums, workers’ comp premiums, 401(k) plan costs, and onboarding separately from the fee. Statutory costs remain yours as well. You pay UI and the related DUA contributions on the first $15,000 of wages. With 25 or more covered individuals, you also fund the employer share of PFML. Add federal FICA and FUTA and your workers’ comp premiums, and you have the full bill. Total cost per employee per year is the only fair comparison.

INSIGHT: What should Massachusetts payroll services handle for you?

Payroll here has more employer programs than most states. Ask a provider to walk you through all of them.

  • UI wage base of $15,000, with Workforce Training Fund, EMAC, and COVID-19 Recovery Assessment.
  • PFML at 0.88% or 0.46%, depending on employer size.
  • Earned sick time: 1 hour per 30 hours worked.
  • Minimum wage: $15.00 an hour.

Many Bay State companies have retirees turned consultants or a sales office in Florida. That state has no income tax and no paid leave program, so payroll looks very different. My guide to PEO companies in Florida explains who serves it.

Technology and life sciences firms here often hire on the West Coast too. California layers on state disability insurance, daily overtime, and its own sick leave law. Start with my overview of PEO companies in California if that applies to you.

FAQs on PEO and Payroll Services in Massachusetts

What does a PEO do for a small business in Massachusetts?

Through a signed agreement, the provider assumes your employer status. That allows it to run payroll and file state and federal employment taxes. It can offer group health and retirement plans, provide workers’ compensation coverage, and manage HR paperwork such as sick time tracking and leave contributions. You stay in charge of hiring, firing, and daily work. The service agreement sets out which responsibilities the provider takes on and which stay with you. State law requires that agreement to say who provides workers’ compensation.

Do PEOs have to register in Massachusetts?

Yes. A co-employment provider must register with the Department of Labor Standards before operating here, and it must renew annually. It must maintain positive working capital, file audited financial statements, and post a $250,000 surety bond. Clients must give employees a notice that explains the arrangement. The department publishes a list of registered providers on its program page, so you can check a name before signing. IRS certification is a federal matter and is no substitute for state registration.

How much is the Massachusetts PFML contribution in 2026?

For employers with 25 or more covered individuals, the contribution is 0.88% of eligible wages. Employers may withhold part of it from employees’ pay, and the employer pays the rest directly. Smaller employers remit only 0.46% because they don’t owe the employer share of the medical leave portion. Both rates are unchanged from 2025. A 2026 law shifts employer contributions from medical leave to family leave starting January 1, 2027. Payroll settings will need another look at year-end.

Is paid sick time required for Massachusetts employers?

Yes. Sick time accrues at a minimum of 1 hour per 30 hours worked, and employees can earn up to 40 hours per year. Employers with 11 or more employees must pay for that time. Smaller employers must still provide it, but it may be unpaid. The time is job-protected, and workers can use it for themselves or certain family members. Track accrual in your payroll system from day one, because the 11-week count includes full-time, part-time, and temporary staff.

How much does a PEO cost in Massachusetts?

You will be quoted either a monthly amount for each employee or a percentage of your total payroll. Most providers quote case by case, based on headcount, industry, and the benefits you choose, though a few publish rates. Health premiums, workers’ compensation premiums, and retirement plan costs come on top of the service fee. Statutory contributions, such as unemployment insurance and the employer share of paid leave, stay with you. Compare proposals on total annual cost per employee, not on the advertised fee.

What is the minimum wage in Massachusetts for 2026?

The minimum wage is $15.00 an hour, unchanged since January 1, 2023. Tipped employees may be paid a service rate of $6.75 an hour if tips bring them to at least $15.00. When tips fall short, the employer makes up the gap. Work beyond 40 hours in a week must be paid at 1.5 times the regular rate. Proposals to raise the minimum come up regularly, so confirm the current rate with the Attorney General’s office each January before running payroll.
